We accept most major insurance and many others. Please give our office staff a call to check if you’re covered.

To ensure a smooth appointment, please confirm the date and time and arrive promptly. For your initial visit, please bring a photo ID and insurance card if applicable. Additionally, any relevant medical information such as medical reports, electro diagnostic studies, diagnostic test results, and actual films, if available, should be brought. If you have previously seen a pain specialist, please provide a list of procedures and/or a copy of office notes. If required by your insurance, a medical referral is necessary. For cases related to an automobile accident or workers’ compensation, insurance information and adjuster contact details are required.

 

In case you are unsure of your current medications, please bring them with you. It is important to provide the name of the referring individual for our records.

 

Upon arrival, you will be provided with forms to complete. However, to expedite the process, personal information and complaint questionnaires can be downloaded from our website and completed prior to your appointment.

If it is not possible to attend the scheduled appointment, please notify us by calling in advance. We are more than happy to reschedule your appointment for another day. In case of delay, please contact the office as soon as possible.

We offer a wide variety of treatments including, minimally-invasive-spinal-surgery, nerve-block-injection, spine-injections, joint-injections, platelet-rich-plasma-therapy and more. Please give our office staff a call for more information.

To prepare for the procedure, it is crucial to abstain from eating for a minimum of six hours. If the procedure is scheduled for the morning, do not consume any food or drink after midnight. If you have high blood pressure, you may take your medication at the scheduled time before the procedure with a small sip of water. The need to take medication for other specific conditions will be discussed with you prior to the procedure.

 

On the day of the procedure, you will not be able to drive, and it is essential to bring an escort with you. Without an escort, we will be unable to perform the procedure. If there have been any recent diagnostic studies conducted, kindly bring the results and/or report with you. Dressing in comfortable clothes is recommended.

Following a brief interview, a gown will be provided for you to change into. Your personal belongings will be securely stored in an individual locker. Before the procedure, the doctor will review your medical history and current condition being treated. In the procedure room, monitors will be attached to check your blood pressure and heart rate. The treatment area will be sterilized, and if anesthesia is required, you will be sedated. After the procedure, you will be monitored in a private area until you are fully conscious. Upon evaluation, and brief exit interview, your escort will be responsible for taking you home. Follow-up instructions will be provided.
